Medication Management Service
Service Details

Our pharmacists provide medication users and their caregivers with medication assessments, recommendations, and follow-up consultations at either a face-to-face meeting or at a telemedicine appointment for free. Our pharmacists’ service include:

Evaluation of patients’ health status, medication usage, and self-care capabilities.
Collaboratively review and integrate treatment regimens, including indications, potential side effects, and precautions.
Discussing important considerations for new or high-risk medications.
Developing a personalized medication management plan in partnership with a patient and her/his caregiver.
Consolidation of medication records to create or update individual’s medication lists.
Giving advice to prescribers to identify and resolve potential drug conflicts and enhance therapeutic outcomes.

Our pharmacists may access the eHealth Electronic Health Record Sharing System to access information for better understanding of the conditions of individual medication users.

Our team shall contact you within 1 to 3 working days upon receiving your application.

Schedule the initial consultation date and time (consultation duration approximately for one hour; the actual duration depends on the individual needs of the participant).

Follow-up appointments shall be arranged as necessary, with ongoing support available via the pharmacist hotline service.

For those in need, you may also visit the pharmacy in person, call us, or contact us via WhatsApp message during opening hours.
